Description of requirement and submittal requirements are below Description: The Contractor shall provide for Healthcare for Homeless Veterans (HCHV) Contracted Emergency Residential Services (CERS) Emergency Housing program provides care, treatment and rehabilitative services to homeless Veterans in San Joaquin County, CA. The services provided to eligible homeless veterans include outreach, case management and housing services in accordance with the PWS. Reference the draft PWS for additional details.
